IPA form 一共有六张， 你的雇主给你几张？
一般上你要不要体检都有写在IPA form的第二章的第13条， 你去看下就知道了。。
给我的是一个PDF文件里面有标的是page x/8 ,但一共是6页。
第三页写着需要的文件有
1 IPA
2 PASSPORT
3 The original and completed Declaration Form duly signed by the pass holder,local Employer/Sponsor and if applicable,the Third Party user who submitted the application on behalf of the local employer/sponsor.
然后在要签字的上面有写一行字
I further declare that I have not suffered or am not suffering from AIDS or infected with HIV or Tuberculosis. …….
你先说你的是什么pass， WP， SP， EP。。。。。
你以前做过体检吗？三个月之内做过是不用再做的。

SP/EP和WP的IPA是不一样的
既然上面没有说就不用做。
